  1. It was Emily’s first visit to the clinic. She went to the counter, where the staff assisted her in filling out a form with personal information and medical history. If she had any chronic conditions or was taking any medications, the staff reminded her to mention them.Emily felt a bit congested and began sneezing. She told the doctor about her running nose and that her throat felt a little sore. The doctor decided to prescribe some over-the-counter medication to help alleviate her symptoms.
If Emily wears glasses for close-up reading, she would mention that she needs nearsighted glasses. Additionally, Emily’s family has a history of diabetes, which the doctor noted. The staff also asked about her occupation and whether she frequently works night shifts, as this could affect her health.

John is a retired construction worker who was recently diagnosed with diabetes. Since then, he has been careful about his diet and medications. However, last week, he developed a dry cough with some phlegm.

“Like I said, I don’t get sick often,” John told his doctor, “but this cough is really bothering me.”

The doctor explained that his condition might be caused by a minor infection or irritation. “Just in case, I’ll prescribe some cough syrup and vitamins to help boost your immune system. You can also take some over-the-counter medication if the symptoms persist.”

John thanked the doctor and went home to rest. He knew that managing his disease properly was important, especially since any minor illness could affect his overall health. As someone who had gone through tough times in his occupation, he was determined to take care of himself and avoid any serious complications that might require an operation in the future.

At the clinic, you need to fill out a form with your name, medical history, and if you have things like diabetes. You also write down your medicines and why you came. If you had any operations or if you are retired, you tell them too. The doctor may ask about your family and ask you to give more details. If you say you have a sore throat, feel congested, or have a wet cough with phlegm, they might give you syrup or tell you to take vitamins.

Sarah, a nurse working the night shift at a busy clinic, had been feeling a bit under the weather lately. She had a dry cough and a sore throat that wouldn’t go away, and her nose had been congested. Despite these symptoms, she continued her shift, working diligently to care for her patients.

One night, during a routine check-up, an elderly man came in with a prescription medication refill request. His wife, concerned about his chronic condition of mild diabetes, reminded him to take his syrup and check his blood sugar regularly. Sarah handed him the medicine, making sure he understood how to use it properly.

The man’s wife also mentioned that he had been using reading glasses for a few years now. Sarah, who had nearsighted glasses herself, smiled and offered some advice about eye health.

Before they left, Sarah quickly jotted down some details on the clinic’s personal information form, making sure everything was up to date. As the couple left, she took a moment to step outside and grab a quick drink from the drug store nearby. The air was chilly, but she didn’t mind; she needed a little break after the long shift.

Back at the clinic, Sarah filled out the last few forms for the evening. A quick glance at the clock reminded her that it would be another graveyard shift tomorrow night.

  10. Yesterday, I went to the clinic because I was experiencing congestion, sneezing, and a sore throat. When I arrived, I was asked to fill out a clinic form with my personal information, medical history, and any chronic conditions I had. I wrote that I have no chronic conditions, but I listed my current medications, including a vitamin supplement I take daily.

The doctor asked about my family history and if anyone in my family had diabetes or any other disease. Then, I explained the reasons for my visit, mentioning that I had a productive cough and a runny nose, and that I had to blow my nose frequently. The doctor checked my throat and prescribed a syrup to help with my cough.

Since my symptoms were not too serious, the doctor suggested I take some over-the-counter medication for decongestion and rest for a few days. Before leaving, I asked if I should return for another check-up, and the doctor said, “Just in case your symptoms get worse, come back.”

Now, I’m resting at home and hoping to feel better soon!

On my way home, I felt like I was getting a cold. My nose was congested, and I had to blow my nose a lot. I also started sneezing. I stopped by a store to buy some tissues, just in case I needed more later.

When I got home, I took some over-the-counter medication to feel better. Then, I put on my reading glasses and started reading a book before bed.

I work as a nurse, and I often see patients with diabetes. This disease requires careful management, including medication, syrup, and vitamins.

Yesterday, I assisted in an operation for a diabetic patient. Afterward, I reminded them to take care of their health just in case complications arise.

Recently, a senior nurse decided to retire after 30 years. Like I said, this occupation is challenging but very rewarding.

When the weather changes drastically, my allergy always flares up, causing sneezing, a runny nose, and the need to constantly blow my nose. This time, I also have a sore throat and a productive cough (wet cough), and the phlegm makes me feel really uncomfortable.

I checked my medicine/medication at home and realized I was out of decongestion, so I had to go to the drug store to restock and buy some vitamins to boost my immune system. Hopefully, this won't last too long so it won’t affect my daily life and work!

When I arrived at the clinic, the receptionist handed me a clinic form to fill out. It asked for my personal information including my name, address, and occupation, along with details about my medical history and any chronic conditions. I had to mention whether I was currently on prescription medication or taking any over-the-counter medication.

Since I had been feeling congested lately, with a running nose, sneezing, and a sore throat, I marked the reasons for visit as related to my symptoms. The doctor later reviewed my form and asked about my family history, especially concerning serious illnesses. Like I said, I had a mild case of diabetes and mentioned that my father had suffered from the same condition.

The doctor suggested I take a decongestion syrup to relieve the symptoms and recommended that I blow my nose gently to help with the phlegm buildup. I was also advised to use allergy medication, since these symptoms could be caused by seasonal allergies. Before leaving, the doctor prescribed a few medications, including a vitamin supplement and some syrup for the congestion.

After the appointment, I went to the pharmacy to pick up my prescription medication. The pharmacist also gave me a box of contact lenses as I had run out of them. I asked for some reading glasses as well, since I often struggle with close-up reading. Just in case, I decided to get some farsighted glasses for long-distance vision.

Now that I had everything I needed, I felt prepared to tackle the symptoms and keep my health in check. With the right medication and careful attention to my condition, I would soon feel better.

Cathy visited the clinic because she had a sore throat, congestion, and a productive cough. The doctor asked him to fill out a clinic form with his personal information, medical history, and current medications.
After checking her symptoms, the doctor prescribed a syrup and some over-the-counter medication to help with her cough and congestion. Cathy mentioned that she had diabetes, so the doctor was careful about her treatment. Before leaving, the doctor reminded her to take her prescription medication as instructed, just in case his condition worsened.

  45. N: Hello! Have you been to our clinic before?
    P: Yes, I have.
N: Great! Please fill out this clinic form. You only need to write down your present illness.
P: Oh no, I forgot to bring my contact lenses, and I just had an operation on my right hand. I can’t write.
N: No problem! Just tell me, and I’ll write it down for you.
P: I started having a running nose two days ago. That morning, I also had a sore throat. Yesterday, I felt congested, so I picked my nose.
    N: Oh, I see.
P: Then, I got a nosebleed. I went to a drug store and bought some syrup and vitamins, but they didn’t work. That’s why I came to see the doctor.
N: I understand. By the way, you look really tired. Are you okay?
P: Oh, I’m on a graveyard shift… I haven’t had much sleep. And to make things worse, I’m allergic to dust mites, so I’ve been sneezing all night.
P: Looks like we both really need to see a doctor!
